Glaucoma is one such condition. It can progress silently for years while gradually damaging the optic nerve, which carries visual information from the eye to the brain. By the time a person begins to notice changes in vision, substantial damage may already have occurred, and vision lost to glaucoma generally cannot be restored. This is why early detection, rather than waiting for symptoms, is central to protecting sight.<\/p>\n<p>Glaucoma is better understood not as a single disease but as a group of conditions that damage the optic nerve. In many cases, impaired drainage of fluid inside the eye causes intraocular pressure to rise, increasing the risk of optic nerve damage. However, glaucoma can also occur in people whose eye pressure falls within the normal range. Measuring pressure alone is therefore not enough to rule out the disease. The condition of the optic nerve, the visual field and other relevant findings must be assessed together.<\/p>\n<p>The most common form, open-angle glaucoma, usually develops gradually. In its early stages, there may be no obvious symptoms. As the disease progresses, peripheral vision is often affected first. Because central vision may remain clear, a person may not realise that the outer edges of the visual field are slowly narrowing. Significant difficulty in everyday activities may appear only when the disease has advanced considerably. For this reason, glaucoma is more effectively detected through regular comprehensive eye examinations based on individual risk rather than by waiting for visual symptoms to emerge.<\/p>\n<p>Not all forms of glaucoma progress silently. Acute angle-closure glaucoma can cause a sudden and severe rise in eye pressure. Intense eye pain, redness, blurred vision, coloured halos around lights, headache, nausea or vomiting may be warning signs. This is an eye emergency. Delayed treatment can result in permanent visual damage, making immediate medical attention essential.<\/p>\n<p>The risk of glaucoma generally increases with age. A family history of the disease, elevated eye pressure, previous eye injury, certain anatomical characteristics of the eye and prolonged use of some steroid medications can also increase risk. Diabetes and some other chronic health conditions may also be associated with a greater likelihood of developing glaucoma. People with these risk factors should not wait until their vision deteriorates before seeking an eye examination.<\/p>\n<p>A glaucoma assessment involves much more than checking whether someone needs spectacles. An eye specialist may measure intraocular pressure and examine the optic nerve. Depending on the situation, visual-field testing, assessment of the drainage angle and detailed imaging of the optic nerve and retina may also be required. Modern diagnostic techniques make it possible to compare very small structural changes over time. In glaucoma care, the pattern of change across repeated examinations can be more important than the result of any single test.<\/p>\n<p>Once glaucoma is diagnosed, the primary goal of treatment is not to restore vision that has already been lost, but to preserve the vision that remains. Eye drops that lower intraocular pressure are commonly used as initial treatment. Depending on the type of glaucoma, the level of pressure and the extent of optic nerve damage, laser treatment or surgery may be required. The objective is to prevent or slow further damage to the optic nerve. When glaucoma is detected early and treatment is followed consistently, useful vision can often be preserved for many years.<\/p>\n<p>One of the major challenges in glaucoma treatment is adherence. A patient may not notice any immediate benefit after using prescribed eye drops because the disease often has no obvious symptoms at the time treatment begins. This can lead some people to stop medication on their own because they feel their eyes are normal. Such a decision can be dangerous. Using prescribed drops regularly, attending follow-up appointments and repeating necessary tests are all essential parts of treatment.<\/p>\n<p>A normal eye-pressure reading should also not be interpreted as proof that the risk has disappeared. Glaucoma and high eye pressure are not the same condition. Some people develop glaucomatous optic nerve damage despite having pressure within the normal range, while others may have elevated pressure without immediate evidence of glaucoma. The overall condition of the optic nerve, visual field and disease progression is therefore more important than a single pressure reading.<\/p>\n<p>In Nepal, greater attention is needed to chronic eye diseases such as glaucoma, cataract and diabetic retinal disease, particularly as the population ages. The common habit of seeking eye care only after vision becomes noticeably poor is especially risky in glaucoma. People who are older, have a family history of glaucoma, have elevated eye pressure or possess other recognised risk factors should consult an eye specialist about how frequently they need comprehensive examinations.<\/p>\n<p>The greatest danger of glaucoma is not pain but silence. A person may feel completely healthy while damage to the optic nerve continues. The most effective defence is therefore not fear, but awareness. Timely comprehensive eye examinations, accurate diagnosis, consistent treatment and regular follow-up can substantially reduce the risk of permanent visual impairment.<\/p>\n<p>Being able to see clearly today does not necessarily mean the eyes are free from disease.
