Understanding the Risks and Taking Control in Time

Adrian Mercer

Cholesterol is not a poison. The body needs it to build cells and produce certain hormones and bile. Most of the cholesterol the body needs is made by the liver. Problems arise when the balance of fats in the blood is disturbed, particularly when low-density lipoprotein, or LDL, becomes too high. Excess LDL can collect in artery walls and form fatty deposits. Over time, the arteries may narrow or become blocked, increasing the risk of a heart attack, stroke or poor circulation in the limbs.

The danger is that high cholesterol usually causes no symptoms. A person may feel healthy while damage to the arteries develops gradually. Many people learn that their cholesterol is high only through a routine blood test—or after a heart attack or stroke. Waiting for symptoms is therefore not a safe way to detect it.

What should be tested?

A blood test called a lipid profile measures total cholesterol, LDL, high-density lipoprotein, or HDL, and triglycerides. HDL helps carry excess cholesterol back to the liver. Triglycerides are another type of fat found in the blood. A single total-cholesterol number is not enough to determine a person’s risk. Results need to be considered alongside age, blood pressure, diabetes, smoking, family history and any previous heart attack or stroke.

Some tests may require fasting beforehand, but fasting is not necessary for every lipid test. Follow the instructions provided by the laboratory or clinician. How often a person should be tested depends on age, health and risk factors, and should be discussed with a healthcare professional. People with diabetes, heart or blood-vessel disease, a family history of early heart attacks, or previously high cholesterol should ask their clinician when they need testing.

What raises the risk?

Foods high in saturated and trans fats—such as fatty meat, ghee, butter, high-fat dairy products and some commercially prepared foods—can raise LDL. Physical inactivity, excess weight, smoking, heavy alcohol use and an unbalanced diet can also contribute to unhealthy blood-fat levels. Inherited factors may cause cholesterol to become very high. Diabetes, an underactive thyroid, chronic kidney or liver disease, and some medicines may also affect cholesterol. Lifestyle changes matter, but diet is not the cause of every case of high cholesterol.

Very high triglyceride levels also need attention. Levels above 500 milligrams per decilitre may increase the risk of acute pancreatitis, a sudden inflammation of the pancreas. Anyone with a result this high should seek prompt medical advice rather than starting medicines or home remedies without guidance.

Practical ways to manage cholesterol

A heart-healthy diet can include more vegetables, fruit, lentils, beans, whole grains and other fibre-rich foods. Reduce fatty meat, ghee, butter, high-fat dairy products, fried foods and heavily processed foods. Even oils considered healthier should be used in moderation; both the amount of oil and the overall balance of the diet matter. Avoid foods containing trans fats where possible. If you drink alcohol, ask a healthcare professional whether you should reduce or stop.

Regular physical activity supports heart and blood-vessel health. The World Health Organization recommends that adults do at least 150 to 300 minutes of moderate-intensity physical activity each week. Walking is a practical place to start; gradually increase the time and pace. Avoid sitting for long uninterrupted periods, aim to maintain a healthy weight and stop smoking. If a health condition or physical limitation affects your ability to exercise, ask a clinician what level of activity is appropriate.

Are medicines necessary?

Not everyone with high cholesterol needs medication. The decision is based on a person’s overall risk of heart attack or stroke, not on a cholesterol result alone. A clinician may recommend medicine for someone who has already had heart disease or a stroke, has diabetes or other significant risk factors, or continues to have high LDL despite lifestyle changes. Statins are widely used to lower LDL and reduce the risk of heart attack and stroke. Other medicines may be considered when appropriate.

If you develop muscle pain or weakness while taking cholesterol medicine, tell your clinician. Do not stop the medicine or change the dose on your own. A clinician can adjust the dose or consider another option if needed. Supplements marketed as “natural” are not proven substitutes for prescribed treatment, and some may interact with medicines. Seek professional advice before using them.

Do not overlook inherited risk

A family history of very high cholesterol or heart attacks at a young age may point to an inherited condition. It can appear early in life and may not be controlled by a healthy diet alone. Tell your clinician about such a family history and ask whether close relatives should also be tested.

Cholesterol itself does not usually cause sudden chest pain; its effects can build silently over many years. But chest pressure or tightness, difficulty breathing and a cold sweat may signal a heart attack. Sudden facial drooping, weakness in an arm or unclear speech may signal a stroke. Seek emergency medical care immediately if these symptoms occur.

Fear alone does not reduce cholesterol-related risk. Timely testing, understanding the results, practical changes to diet, regular activity and medical treatment when needed can all help. The most responsible first step is to discuss your test results with a healthcare professional and agree on a plan suited to your health and overall risk.

Adrian Mercer Cholesterol is not a poison.
